Please mark day, time and the amount of stress you are feeling before and after your relaxation practice on a 1 to 10 basis, with 1 indicating "no" or "very low" stress and 10 indicating "high" stress.

Also, evaluate the ease or difficulty of the practice you are using so that 1 means it is "easily used" and 10 means it was "very difficult" or "awkward."
